- By pass of discrete areas of obstruction in coronary vessel using the intenal mammary artery, radial artery, areversed segment of sapheneus vein, inferior epigastric artery or gastroepiploic artey
- Internal mammary artey used in 95% of CABGs, usually to LAD
- Three or four graft are used on average
- Minimal invasive CABG (MIDCAB) : fewer incision, no a cardiopulmonary by pass or cardioplegia are used (performed on a beating heart, limited to single vessel disease). Minimizes pain, recovery time and chances of wound infection
- Port access technique : Endovascular aortic occlusion and cardiopulmonary by pass with cardioplegia allow for broader used of MIDCAB (multy vessel disease, combined valve coronary artery surgery)
INDIKASI
- Stable angina refractory to medical therapy
- Unstable angina
- Post- infarct angina
- Double or triple vessel disease with decreased left ventrikuler function
- Left main coronary disease
- During elective valve replacement with critical vessel occlusion
- During surgery for complication of Miocardial infarck
- Complication of percutaneus transluminal coronary angioplasty (PTCA)
CONTRAINDICATION
- Cronic CHF
- Ischemic Cardiomyopathy with no sign of angina or reversible ischemia
